27 weeks with twins stress and contractions?

I am 27 weeks with twins. I have sciatica and have been instructed to work 20 hours a week from home. my job hower doesn't allow me to turn it off after my 4 hours. tonight I had a problem withone of y workers that escalated. I got upset and started with a tightening feeling at the top of my stomach. I started with menstral type pains on my whole stomach for almost 4 hours now. I called dr and after an hour she called back to say relax and drink a glass of wine. should i ingore her and go to hospital? she won't take me out of work since there isn't a medical reason to do so. I will have to continue to handle the situation that began tonight tomorrow. I don't think i can do it.
